Clinical Pharmacist - Acute Care

Schedule: 1st shift 7:00am-3:30pm; rotating shifts

Novant Health is seeking a Clinical Pharmacist, to join our pharmacy team, to deliver optimal medication management across all dimensions of care, every time. The Clinical Pharmacist serves as part of the healthcare team at Novant Health by supporting authentic personalized relationships with patients and their chosen support system, collaborating with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health. The pharmacist accepts responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures.

What You'll Do

Essential functions include:

  • Flexibility in working in acute care capacities with adults and pediatrics (central distribution, IV room, med/surg, and critical care)
  • Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards
  • Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems,
  • Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care
  • Makes appropriate evidence-based, patient- centered medication recommendations
  • Participates in the management of medical emergencies for adult and pediatric patients
  • Provides discharge medication review, reconciliation, and counseling as appropriate
  • Maintains proficiency in and actively engages in central dispensing functions and operational programs
  • Oversees pharmacy operations and technician practice/activities
  • Maintains proficiency in preceptor roles and actively emulates education/ mentorship skills


What We're Looking For

  • Education: High School Diploma or GED, required. 4 Year / Bachelors Degree Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. PharmD, preferred.
  • Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy. Oncology experience preferred for oncology roles, required. For Infectious Diseases/Antimicrobial Stewardship roles: ASHP - accredited PGY2 Infectious Disease residency OR minimum of 2 years of equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship, required.
  • Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role, required.
  • Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
